[Mathreu] questionnaire

Hi all,

We are drawing close to the end of this summer's REU and would like to hear
your thoughts on how things went.  Please take a few minutes to fill out the
attached questionnaire and return it to Kris Hunnicutt as soon as possible.
I have attached both latex and pdf files, so that you can modify the latex
file if you like.
